Customize Plaque

Want to customize your plaque?  By default, we use the region provided name, voice/instrument, and ensemble.  Select this option to add an extra touch to your personalized plaque. 

Please note: this is a customization and you must have a region plaque in your order.  If not plaque is in your order, your entire order will be cancelled.
